Social Worker - SNF/LTC
Located in Batavia, NY
Salary: $60K - $75K annually - *Based On Experience*
Qualifications:
- Must have BSW or MSW
- Must have current Staff Social Worker experience
- Must have current nursing home experience
- Strong written and verbal skills
- Must have excellent communication skills
Social Worker Job Responsibilities:
- Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living
- Conducting initial and ongoing evaluations of residents' emotional, social, and cognitive needs to identify potential issues.
- Develope individualized care plans that address the unique needs and goals of each resident
- Provide information to the resident, and/or family member, on residents' rights and responsibilities
- Provide support and counseling to residents and families experiencing loss.
- Knows how to identify and connecting residents to community resources and support groups.
- Educating facility staff on resident needs and how to best support them socially and emotionally.
