Social Worker

We are seeking a dedicated and skilled Social Worker (LMSW/LCSW) to join our hospital-based care team. In this role, you will provide comprehensive psychosocial assessments, clinical support, crisis intervention, and discharge planning for patients and families across diverse medical units. Social Worker collaborates closely with an interdisciplinary team to ensure safe transitions of care and promote emotional well-being during hospitalization.

MAJOR TASKS, DUTIES AND RESPONSIBILITIES

KEY RESPONSIBILITIES:

1. Clinical Assessment & Intervention

A. Conduct comprehensive psychosocial assessments for patients with complex medical, emotional, or social needs

B. Provide therapeutic interventions such as brief counseling, crisis de-escalation, grief support, and motivational interviewing

C. Identify risk factors including abuse, neglect, and suicidal ideation, and follow all mandated reporting requirements

2. Care Coordination & Discharge Planning

A. Develop and implement individualized discharge plans in collaboration with physicians, nurses, and case managers.

B. Coordinate post-acute referrals, including home care, rehabilitation, behavioral health, long-term care, and community services.

C. Communicate with insurance providers, external agencies, and support networks to ensure safe and timely transitions of care.

3. Patient & Family Support

A. Assist patients and families with navigating medical decisions, advance care planning, and end-of-life discussions.

DOCUMENTATION, COMPLIANCE & REGULATORY STANDARDS:

1. Document all assessments, interventions, and discharge plans accurately and promptly in the electronic medical record.

2. Follow hospital policies, the NASW Code of Ethics, and all state and federal regulations.

3. Ensure all work aligns with Joint Commission, Department of Health (DOH), and Centers for Medicare & Medicaid Services (CMS) guidelines.

4. Maintain compliance with quality, safety, and accreditation standards.

COLLABORATION & PROFESSIONAL PARTICIPATION:

1. Attend required department meetings, interdisciplinary rounds, care conferences, and hospital training courses

2. Collaborate with nurses, physicians, case managers, and external partners to support patient care

3. Participate in quality-improvement initiatives and professional development activities

QUALIFICATIONS

1. Master’s degree in social work (LMSW/LCSW) from an accredited institution

2. Current LMSW/LCSW license

3. Experience in healthcare, acute care, or behavioral health preferred

4. Strong crisis intervention skills, communication abilities, and patient-advocacy experience

5. Knowledge of medical terminology, community resources, and discharge planning processes

6. Ability to work effectively in a fast-paced, multidisciplinary environment

EDUCATION: The social worker must have a minimum of BSW from an accredited social work program.

EXPERIENCE: 3 years’ experience in a clinical setting and/or ambulatory care setting preferred.

Sound computer knowledge and skills including an aptitude for using health information technology to guide care management activities and functions. Ability to work independently and meet deadlines. Creativity and strong organizational skills.
