Clinical Social Worker – Care Management
Victorville, CA (Onsite Local Travel to Oak Hills. CA)
About the Role
We are seeking a dedicated Clinical Social Worker to provide comprehensive care management services within a mission-driven healthcare organization. In this role, you will support an interdisciplinary team (IDT) while delivering high-quality, patient-centered care to a vulnerable population.
Key Responsibilities
Care Management (70%)
- Conduct psychosocial assessments, screenings, and ongoing care planning
- Participate in IDT meetings (initial, 6-month, and annual assessments)
- Provide counseling, crisis intervention, and safety planning (including high-risk cases)
- Coordinate care and connect patients with community resources and services
- Advocate for participants and support care plans addressing mental health, social, and behavioral needs
- Lead group counseling sessions and support groups
- Support discharge planning and transitions of care
- Perform occasional home visits (local travel required)
Team & Program Support (25%)
- Collaborate with clinical teams to support care coordination and program goals
- Assist with new patient enrollment and care transitions
- Participate in quality improvement initiatives and case conferences
- Provide consultation to staff regarding psychosocial factors impacting care
Additional Duties (5%)
- Complete additional projects and responsibilities as assigned
Qualifications
- Master’s Degree in Social Work (MSW) required
- Minimum 2 years of experience in a healthcare or related setting
- At least 1 year working with the elderly population (within the last 3 years)
- CPR & First Aid certification (or ability to obtain within 6 months)
- Valid driver’s license and reliable transportation (travel up to ~30%)
Preferred Qualifications
- Experience in care management or case management
- Bilingual in Spanish or another threshold language (Arabic, Farsi, Chinese, Korean, Russian, Vietnamese)
