Matriarch Home Care Concierge is seeking a Licensed Practical Nurse (LPN) to provide one‑on‑one, in‑home nursing care for clients in the Antioch area. This role is ideal for an LPN who values autonomy, relationship‑based care, and the opportunity to make a meaningful difference outside of traditional facility settings.

What You'll Do
As an In‑Home LPN, your focus will be on skilled nursing care and clinical support within the client's home.
Responsibilities include:
- Perform basic nursing assessments and ongoing health monitoring
- Administer medications and treatments as ordered
- Assist with chronic condition management and daily health needs
- Observe and document changes in condition and report appropriately
- Support client safety and overall well‑being in the home
- Communicate clearly with families, RNs, and care management staff
- Maintain accurate, timely clinical documentation
- Follow all care plans, policies, and standards of practice
