Agawam South RehabNursing is seeking a detail-oriented and experienced Licensed Practical Nurse (LPN) to join our team as an MDS Assessment Coordinator in our skilled nursing facility.
In this role, you will coordinate and complete MDS assessments to support accurate care planning and optimal reimbursement. You will work collaboratively with the interdisciplinary team to ensure timely, compliant, and precise clinical documentation.

Key Responsibilities
- Coordinate, schedule, and complete MDS assessments in accordance with state and federal guidelines
- Gather and validate clinical data from nursing, therapy, and medical records
- Ensure accuracy of documentation to support proper reimbursement
- Participate in care plan development and interdisciplinary team meetings
- Monitor case mix index (CMI) and identify opportunities for improvement
- Maintain compliance with CMS regulations and facility standards
Qualifications
- Active Massachusetts LPN license (required)
- Previous MDS experience in a skilled nursing facility
- Strong understanding of MDS processes, scheduling, and submission
- Long-term care or rehab experience preferred
- Strong attention to detail and organizational skills

- Salary: $91,000 - $95,000 annually (DOE)
- Salary adjustments tied to census
- Quality Measure Bonus: Up to $7,500/year
