Agawam South RehabNursing is seeking an experienced LPN to join our team full-time as an MDS Assessment Coordinator in our skilled nursing facility.
In this role, you will coordinate and complete MDS assessments to support accurate clinical documentation, care planning, and reimbursement. You will work closely with nursing, therapy, and interdisciplinary teams to ensure timely and compliant assessments.
Responsibilities
- Coordinate and complete MDS assessments and related documentation
- Ensure timely and accurate submission of assessments
- Collaborate with interdisciplinary teams on resident care planning
- Support reimbursement and regulatory compliance through accurate documentation
Qualifications
- Active Massachusetts LPN license required
- Previous MDS experience in a skilled nursing facility preferred
- Understanding of MDS scheduling, coordination, and completion
- Long-term care or rehab experience preferred

- Salary: $91,000 - $95,000 annually (DOE)
