Role And Responsibilities
Provide direct nursing care to acutely and chronically ill patients in their homes, following physician prescriptions.
Document patient visits thoroughly in electronic medical records and electronic visit verification systems.
Participate in care coordination, communicate effectively with patients, families, physicians, and healthcare team members.
Adapt to diverse home environments and ensure adherence to infection control procedures.
Work under the supervision of a Registered Nurse or designated supervisor, ensuring high-quality patient care.

Qualifications And Skills
- Current, unrestricted LPN licensure in the practicing state.
- Valid driver’s license, vehicle insurance, or reliable transportation.
- Preferred: CPR certification, ability to work flexible hours, strong communication and organizational skills, ability to work independently, and manage multiple tasks.
- Deliver clinical services within the scope of practice and in accordance with the plan of care.
- Coordinate care activities and report patient needs and changes promptly.
- Maintain accurate documentation and comply with regulatory requirements.
