Role: Hospice LPN
Responsibilities: Provide hands-on clinical care in patients' homes, including treatments, wound care, medication administration, and monitoring patient conditions. Assist in developing and adjusting individualized care plans, collaborate with healthcare professionals, educate patients and caregivers to promote self-care, and accurately document all care activities. Participate in team meetings and interdisciplinary coordination to ensure quality patient support.
Qualifications & Skills: Valid Rhode Island LPN license, equivalent education for licensure, at least 1 year of clinical experience (home health or acute care preferred), current CPR certification, valid driver’s license with auto insurance, and strong clinical, communication, and documentation skills.
