- Provide direct nursing care to acutely and chronically ill patients in their homes, following physician’s care plans.
- Document patient visits accurately in electronic medical records and visit verification systems.
- Participate in care coordination by reporting patient needs, attending case conferences, and communicating with patients, families, and healthcare team members.
- Adapt to various home environments, ensuring quality care regardless of patient demographics.
- Unrestricted LPN licensure in the practice state.
- Valid driver’s license, vehicle insurance, and access to reliable transportation or public transit.
- Preferred: CPR certification, ability to work flexible hours, manage multiple tasks, work independently, and possess strong communication and organizational skills.
