DUTIES AND RESPONSIBILITIES:
Provide Care Coordination field-based services to assigned client population
Provide care coordination to alleviate identified needs related to behavioral,
physical, and social issues.
Focus on addressing issues that will decrease medical costs and increase
quality of life.
Develop positive relationships with clients, care givers, providers, and other
team members.
Interact with peers in a collegial manner, supporting efforts to improve
outcomes.
Managing physical health problems through provision of education
Connecting with other community services
Identification of dietary concerns
Home safety evaluations
Safe medicine use
Education on specific illnesses and treatments
Stress management
Emotional support
Monitoring process of clients through home visits
Compliance with HIPAA regulations
Maintain accurate records of care coordination in the EMR
