Job Title: Pharmacist

Job Description

We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for our patients. This position requires strong clinical skills, excellent patient service abilities, and a commitment to providing high-quality pharmaceutical care in a hospital or managed care setting.

Responsibilities

• Review and interpret medication orders to ensure accuracy and appropriateness for patient therapy.
• Counsel patients on medication usage, side effects, and interactions to promote adherence and understanding.
• Collaborate with healthcare professionals in acute care settings to optimize pain management and medication therapy.
• Conduct medication reconciliation during patient admissions and discharges to prevent errors.
• Monitor patient responses to medications and adjust therapy as needed based on clinical guidelines.
• Maintain comprehensive knowledge of medical terminology, anatomy, and pharmacy practices.
• Participate in pharmacy residency programs or ongoing education to enhance professional development.

Essential Skills

• Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
• Active pharmacist license in the state of practice.
• Experience in hospital medicine or managed care is preferred but not required.
• Strong knowledge of acute care practices and pain management strategies.
• Excellent communication skills with a focus on patient service.
• Ability to work collaboratively within a multidisciplinary team.
• Familiarity with medical terminology and anatomy knowledge is essential.

Additional Skills & Qualifications

• Completion of a pharmacy residency is highly desirable.
• Must be willing to float across NYC.
• Experience with prior authorization processes.

Work Environment

Retail and Outpatient Hospital Setting. Flexible PTO and benefits available upon request.

Pay and Benefits

The pay range for this position is $65.00 - $68.50/hr.
