Key Responsibilities:
- Serve as the contact point, advocate and informational resource for clients, care team,
- family/caregivers and community resources regarding client's service plan
- Liaison between clients, landlords, and DCA to ensure family maintains good tenancy
- Facilitate housing searches for new enrollments and relocations as needed
- Assist with completion of section 8 voucher applications and re-certifications
- Partner with clients to monitor adherence to their service plan, evaluate effectiveness, monitor progress in a timely manner and facilitate changes as required.
- Facilitate client linkages, referrals and access to appropriate treatment, community supports and medical or specialty providers according to the service plan
- Cultivate and maintain relationships with external community providers and supports.
- Completes and submits accurate reports and chart documentation in a complete and timely manner according to program guidelines and established procedures.
- Ensures quality and accuracy of written and verbal communications.
- Support the Quality/Compliance/Risk function as required.
- Performs other related duties as assigned
Requirements:
- Bachelor's level practitioner with at least 2 years of experience in social service-related or housing related field preferred.
- Must have and maintain a valid driver's license with an acceptable driving record
- Must be able to exercise independent thinking and good judgment under all circumstances.
- Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.
Additional Information:
- Base Pay: $19.23/hourly
- Bilingual Rate: An additional $1.50/hourly rate added to the base pay if Spanish proficient
- Schedule: Days
