Key Responsibilities: Serve as the contact point, advocate and informational resource for clients, care team, family/caregivers and community resources regarding client's service plan Liaison between clients, landlords, and DCA to ensure family maintains good tenancy Facilitate housing searches for new enrollments and relocations as needed Assist with completion of section 8 voucher applications and re-certifications Partner with clients to monitor adherence to their service plan, evaluate effectiveness, monitor progress in a timely manner and facilitate changes as required. Facilitate client linkages, referrals and access to appropriate treatment, community supports and medical or specialty providers according to the service plan Cultivate and maintain relationships with external community providers and supports. Completes and submits accurate reports and chart documentation in a complete and timely manner according to program guidelines and established procedures. Ensures quality and accuracy of written and verbal communications. Support the Quality/Compliance/Risk function as required. Performs other related duties as assigned Requirements: Bachelor's level practitioner with at least 2 years of experience in social service-related or housing related field preferred. Must have and maintain a valid driver's license with an acceptable driving record Must be able to exercise independent thinking and good judgment under all circumstances. Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.
