Job Overview:
The case manager develops and monitors individualized treatment plans and needs assessments while connecting clients with appropriate community resources and advocating on their behalf. They maintain clear, professional communication with supervisors, referral sources, and external partners, offering timely feedback and addressing concerns or training needs as they arise. In this role, the case manager consistently upholds agency standards by providing responsive, supportive customer service and representing the organization with professionalism.

Program Info
Involuntary Outpatient Commitment (IOC) serves individuals diagnosed with serious, persistent mental illness who, due to his/her mental illness are dangerous to him/herself, others or property in the reasonably foreseeable future. These individuals have also demonstrated a history of positive treatment response in an outpatient setting. IOC is an opportunity to effect change before an individual reaches a crisis point which results in an inpatient hospitalization. The individual must have some level of willingness and capacity to participate in the IOC program and plays an active role in the development of his/her treatment plan.
Services include: case management, service linkage, court system coordination, transportation to IOC related appointments, interaction with family members or other significant social supports, and interim psychiatric medication monitoring for individuals who are not in active treatment at the time of admission. The goal of providing these supports is to increase the individual’s participation and adherence to his/her wellness and recovery plan in order to maintain safety and stability in the community.
Key Responsibilities:
- Creating and monitoring treatment plans & needs assessments, linking clients to community resources, advocating for clients.
- Communicates programmatic or job-related concerns to the supervisor and makes suggestions for improvement in an appropriate and timely manner.
- Communicates any performance concerns or training needs to the supervisor in an appropriate and timely manner.
- Conducts self in a professional manner that positively reflects the agency culture.
- Demonstrates competent written and verbal communication with referral sources and external customers.
- Is attentive and responsive to customer needs in a helpful, supportive and timely manner.
Additional Information:
- Base Pay: Starting hourly rate of $20.19
- Bilingual Rate: An additional $1.50/hourly rate added to the base pay if Spanish proficient.
Requirements:
- The minimum requirement is a bachelor's degree in psychology, social work, or a related field, plus 1 year of relevant experience.
- The role requires meeting clients in the community.
- Must have and maintain a valid driver's license with an acceptable driving record.
