Job Overview
Provide individual and group recovery focused case management services, collaborate with Acenda team members and act as a key partner to providing important input into our delivery of recovery services. Act as a liaison between providers and community agencies that provide supportive services. Provide education sessions, social service connections, and client transportation to support recovery needs.
As an integrated health organization Acenda values the use of evidence-based practice or EBP's.
Program Info
The Adolescent Substance Use Recovery Initiative program is designed to provide treatment and support to adolescents with substance use disorders, focusing on early intervention, skill-building, and long-term recovery. Programs include screening, personalized counseling, family and group sessions, and relapse prevention planning. The goal is to address substance use and associated issues like academic problems or mental health concerns through a comprehensive, family-centered approach.
Key Responsibilities:
- Organize and participate in Family Team Meetings/Child Family Team Meetings
- Support adolescent and family with relevant school-related issues.
- Support connecting adolescent to work or extracurricular activities.
- Facilitate Adolescent/Family engagement and retention
- May provide transportation to program participants for treatment-related activities.
- Function as part of treatment team.
- Documentation in agency’s Electronic Health Record & in CYBER
- Performs other related duties as assigned.
Requirements:
- Bachelor's level practitioner or high school graduate with 3 or more years of experience providing direct care to youth in a behavioral health agency or institutional setting.
- Must have and maintain a valid driver's license with an acceptable driving record
- Must be able to exercise independent thinking and good judgment under all circumstances.
- Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.
Additional Information:
- Hourly Rate: $27.26
- Sign-on Bonus: $500
- Bilingual Rate: $1.50 hourly rate added if Spanish proficient
