We’re looking for a compassionate, motivated Case Manager to join our Home Care team in Holyoke, MA, serving clients in Holyoke, Chicopee, South Hadley, Granby, Ludlow, Belchertown, and Ware. What You’ll Do
As a Case Manager, you’ll be a trusted partner and advocate for consumers in our Home Care program. Your work will include:
- Conducting thoughtful, person-centered assessments to understand each consumer’s strengths, needs, and goals (Visits are conducted in the client's homes)
- Developing individualized care plans that support independence and quality of life
- Coordinating services and community resources to ensure smooth, responsive care
- Monitoring progress and adjusting plans to support the best possible outcomes
- Advocating for consumer rights, needs, and preferences
- Collaborating closely with a supportive, mission-driven team and community partners
Who You Are
You’re empathetic, organized, and driven by purpose. You bring:
- A Bachelor’s degree in Social Work or a related field
- Experience in case management, human services, or working with older adult populations
- Knowledge of community resources and healthcare systems
- Strong communication skills and the ability to build trust with consumers and families
- Comfort with technology, including electronic health records
- A valid driver’s license and reliable transportation
- Ability to successfully complete required background checks (CORI, SORI, SAMs GSA, MassHealth Exclusion List, OIG)
