Provide patient-centered primary and acute care to residents in skilled nursing and post-acute facilities throughout Escanaba, MI, as part of a collaborative, team-based practice focused on older adults.

Schedule and Responsibilities
- Full-time positions with flexible scheduling
- Typical shifts: six-hour facility rounds (start between 7:00–10:00 a.m.)
- No call or overnight responsibilities
- Perform daily rounds, evaluate/manage acute and chronic conditions, develop treatment plans, and prescribe medications
- Efficient documentation using company EHR with AI Scribe and voice dictation on company-provided iPad (LTE)
- Access to UpToDate® for point-of-care guidance

Minimum Qualifications
- Active, unrestricted Nurse Practitioner or Physician Assistant license
- National certification (e.g., ANCC, AANPCB, PNCB, AACN, NCCPA)
- Skilled nursing, long-term care, geriatrics, or post-acute care experience preferred
