Physical Therapist (PT) – Skilled Nursing FacilityLocation: California (SNF Setting)Schedule: 30 hours/week guaranteed | Variable shiftsType: 13-Week Contract | Rehabilitation Therapy Position overviewA Skilled Nursing Facility is seeking a Physical Therapist to provide comprehensive rehabilitative services to adult and geriatric residents with diverse functional and medical needs. This role is ideal for a PT who enjoys working in post-acute and long-term care settings, collaborating with an interdisciplinary team to improve mobility, safety, and quality of life.What you’ll doPerform thorough evaluations of SNF residents, assessing mobility, strength, balance, transfers, gait, and functional abilities to establish individualized plans of care.Deliver skilled physical therapy interventions, including therapeutic exercises, gait training, transfer training, balance activities, and functional mobility training in accordance with physician orders and facility protocols.Monitor patient progress, reassess functional status, and modify treatment plans and goals as clinically indicated.Educate residents and caregivers on home exercise programs, safe mobility techniques, use of assistive devices, and fall-prevention strategies.Maintain accurate, timely clinical documentation that supports billing, regulatory compliance, and quality standards.Collaborate with nursing, occupational therapy, speech therapy, social services, and physicians to support coordinated, patient-centered care.Adhere to SNF policies, state and federal regulations, and professional standards of physical therapy practice.QualificationsActive California Physical Therapist license.Experience in a Skilled Nursing Facility strongly preferred; motivated new graduates are welcome to apply.Strong clinical assessment, treatment-planning, and hands-on intervention skills in geriatric and post-acute populations.Excellent communication and interpersonal abilities for working closely with residents, families, and interdisciplinary team members.Ability to work independently while managing a varied caseload and contributing effectively as part of a rehab team.Role highlightsGuaranteed 30 hours per week with overtime available after 40 hours.Opportunity to build experience in SNF rehabilitation while making a tangible difference in residents’ functional independence and quality of life.
