The care management team operates within a collaborative, unionized environment that emphasizes shared accountability, professional development, and a strong culture of respect and engagement.Key responsibilitiesDeliver comprehensive care management services for hospitalized, at-risk patients, including thorough assessment, planning, implementation, and ongoing evaluation of care plans.Coordinate and facilitate interdisciplinary care, collaborating with physicians, nursing, ancillary services, and external partners to support timely, appropriate, and effective care delivery.Manage resource utilization and level-of-care determinations, supporting appropriate use of hospital services and adherence to medical necessity and regulatory requirements.Lead and document discharge planning activities, ensuring safe, coordinated transitions of care, including post-acute services, follow-up appointments, and patient/caregiver education.Maintain accurate, timely documentation of assessments, interventions, utilization review activities, and care coordination in the electronic health record.Serve as a patient advocate, promoting patient rights, safety, whole-person care, and alignment of the plan of care with patient goals and preferences.Support regulatory and accreditation readiness by practicing in accordance with organizational policies, clinical standards, and applicable state and federal regulations.Required qualificationsAssociate Degree in Nursing or nursing diploma.Current, unencumbered Washington State Registered Nurse (RN) license.Minimum of 2 years of recent acute care experience in a medical-surgical or inpatient hospital setting.Preferred qualificationsBachelor’s Degree in Nursing (BSN) or higher, or active enrollment with completion within 3 years of hire.National certification in case management or related specialty (e.g., CCM, ACM, or specialty nursing certification).At least 1 year of experience in care management, case management, or utilization review in any healthcare setting, or successful completion of an internal care management training/orientation program.What you can expectCaregivers in this department are supported by experienced leadership, care management assistants, and a culture that values professional growth, shared decision-making, and continuous improvement.
