Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and record vital signs such as blood pressure, pulse, and temperature to track health status
- Administer medications accurately following care plans and HIPAA confidentiality standards
- Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary
- Support clients with memory care needs, including Alzheimer's and dementia care strategies
- Use specialized equipment like Hoyer lifts for safe patient transfers and heavy lifting tasks
- Conduct patient observation to identify changes in condition or behavior and report findings promptly
- Maintain cleanliness of the living environment through regular cleaning duties to promote hygiene and safety
- Follow detailed care plans designed by healthcare professionals to ensure consistent, personalized support
- Collaborate with healthcare teams, social workers, and family members to coordinate comprehensive care
Skills
- Proven experience in home health, assisted living, or nursing home environments with a focus on senior or long-term care
- Strong knowledge of patient monitoring techniques and vital signs assessment
- Certified in CPR and First Aid; familiarity with hospice care and working with individuals with disabilities or developmental disabilities preferred
- Excellent communication skills for effective interaction with clients, families, and healthcare providers
- Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
- Experience working with individuals with autism or memory-related conditions such as Alzheimer's disease or dementia
- Knowledge of HIPAA regulations to ensure confidentiality and privacy of patient information
- Skilled in meal preparation tailored to specific dietary restrictions or health conditions
- Ability to follow detailed care plans accurately while maintaining flexibility for changing needs
