Duties:

Conduct thorough assessments and create care plans for both short-term and long-term needs, drawing on your clinical expertise. Actively manage cases based on their intensity and acuity, maintaining continuous communication with members, families, physicians, and the interdisciplinary team. Coordinating resources and advocating for members, ensure their needs are met by assessing care options, utilizing benefits, and engaging community resources. Accurate record-keeping in the electronic health record, following clinical guidelines, is also a key responsibility.

Skills:

Clinical expertise, assessment and care planning abilities, case management skills, effective communication, resource coordination, advocacy, record-keeping, and familiarity with electronic health records.

Education:

• Current NYS Licensure
• 2 Years Care Management Experience
• BLS Required
• CCM preferred but not required.
• Current MLTC care experience managing a case load.

Job Type: Contract

Salary: $50.00 - $58.00 per hour

Expected hours: 40 per week

Schedule:

• 8 hour shift
• Day shift
• Monday to Friday
