Summary

• Department: Behavioral Health

• Shift Timing: 15:00 - 23:30 Monday to Friday 5 days 8 hour shift
• Per Week: 40 Hours

Roles & Responsibilities

• ​Implement and monitor patient care plans. Monitor, record and communicate patient condition as appropriate.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assess and coordinate patient's discharge planning needs with members of the healthcare team.

Minimum Qualifications

​BLS, CPI or equivalent required. inpatient acute care BH required

Job Type: Contract

Pay: $51.00 - $57.00 per hour

Expected hours: No less than 40 per week
