Zion Integrated Behavioral Health Services is Hiring an IHH Care Coordinator Near Atlantic, IA

Overview:

The IHH Care Coordinator will maintain a caseload of patients. This individual will be responsible for referring, monitoring, and coordinating services for adults with a chronic mental illness. 

Essential Duties and Responsibilities:

Maintain a non-threating and welcoming environment while providing professionalism to all individuals seeking services while adhering to readiness and preparedness to the individual being served.

  •  Maintains an active caseload in accordance with clinical quality standards and quality management plan protocols.
  • Conducts in-depth interviews/assessments and use of specialized knowledge to gather complicated information from patients to be enrolled in the IHH.
  • Collaborate with Health Home Team and treating providers in developing and implementing care coordination plan to address treatment barriers for each low to moderate risk patients. Refer high risk patients to Nurse Care Manager.
  • Quality Improvement activities, including data collection, tracking, and analysis.
  • Record and maintain documentation in Practice Management System and verification of health coverage portal.
  • Maintain patient confidentiality according to state and federal regulations.
  • Uses available data, such as exams/assessments scores, participant evaluations, and service observations to identify health wellness needs to develop care coordination plans.
  • Adhere to organization’s policy and procedures and guideline of ethical code.
  • Preforms all other duties assigned.

ADDITIONAL:

  • Oversee Perry office site staff and report to Director and/or CFO
  • Assist in management of grants assigned by the Director.

 

Requirements:

  • Bachelor’s Degree in Social Work, Human Services, or another related field
  • Experience with care coordination of individuals
  • Experience working with individuals with a chronic mental health illness
  • Strong interpersonal and engagement skills.
  • Strong organizational and time-management skills and understanding to meet the organization’s standards for contact with patients.
  • Excellent written and verbal communication skills.
  • Strong computer skills required; experience with EMR system a plus.
  • Ability to support the agency’s mission and philosophy, and demonstrate sensitivity to cultural diversity and workplace harmony.
  • Value for and ability to deliver excellent customer service.
  • Self-directed and flexible to meet the needs of administration and management.
  • Pursue personal development of skills and knowledge necessary for the effective performance of the role.
  • Responsibility for the upkeep, safety and health of office assigned.
