Zepf is Hiring an Assessment Therapist - ACT Near Toledo, OH

Description:

Zepf Center has been serving the Lucas County community for nearly 50 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, Crisis Care, and therapy programs, as well as career development and wellness services. Zepf Center also offers primary care medical services to our patients to contribute to their continuum of care. Zepf Center is a trauma-informed agency and environment for both patients and staff.

We are currently seeking a Full Time Assessment Therapist for our Assertive Community Treatment (ACT) Team. The purpose of the Assessment Therapist is to provide diagnostic assessments, counseling and clinical services of an integrated care group (ACT) in the Care Coordination Department. Essential duties include but are not limited to:

Essential Duties and Responsibilities:

  • Maintain compliance of all integrated care/crisis plans while providing clients with the highest quality of care.
  • Reviews and determines initial eligible of patients to the ACT team.
  • Attends meetings internal and external of the agency in collaboration with potential and existing ACT team clients
  • Complete Assessments on patients in accordance to ACT compliance
  • Perform other duties as assigned to ensure department success.
Requirements:
  • LSW (with Masters), LPC, LISW, LPCC.
  • Degree in Social Work or related field with at least 2 years of clinical experience
  • Must be a current Lucas County Health Officer or eligible within 2 months upon hire.
  • Health management experience and an understanding of data management, managed care and quality improvement strongly preferred.
  • Must possess a basic understanding level of electronic health records and have excellent computer skills.

Those eligible to drive company vehicles must have a valid driver's license and be eligible for coverage as defined by the agency commercial insurance carrier. Those who drive personal vehicles in the course of business must be able to provide proof of insurance.
