Description

Zepf Center has been serving the Lucas County community for nearly 50 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, Crisis Care, and therapy programs, as well as career development and wellness services. Zepf Center also offers primary care medical services to our patients to contribute to their continuum of care. Zepf Center is a trauma-informed agency and environment for both patients and staff.

We are currently seeking a full-time Crisis Coordinator to provide necessary interventions to assist individuals in stabilizing psychiatric symptoms. This position provides direct service to Wood County Law Enforcement officers who are dealing with individuals experiencing a mental health crisis. The Crisis Coordinator will provide case management as well as follow up clinical services, as well as having a case load. You will work in the Zepf Center office and in the field as required providing services. On call all shifts to include evening and weekend work. This is a salaried position.

Essential Duties of this position include but are not limited to:

  •  Provides outreach service including crisis intervention
  • Completes prescreens, crisis plans, lethality risk assessments, and mental status exams. Plans for an completes follow up based on infidel client needs.
  • Interface with law enforcement and other service providers such as Wood County Job and Family Services, Crisis Intervention Team, Hospital Liaison, Wood County Common Pleas court, etc.
  • Provide community-based support and provide services to assigned case management or therapy caseload
  • Gathers diagnostic information for new clients entering the agency. Makes recommendations for services. All clinical works is completed under clinical supervision.
  • Completes initial treatment plan as well as treatment plan revisions and follow up within required time frame.
  •  Provide support to the office to include answering phones, greeting clients, scheduling appointments and other office duties as needed.
  • Gather information about community resources and educate Zepf Center clients and staff
  • Complete documentation in a timely and complete manner as required by the agency, professional license boards, and funder guidelines
  • Works as part of the CIT team in Wood County and complete all board and other required paperwork within the required timeframe
  • Meets agency established productivity standards
  • Carry out policies and procedures of the agency; may participate in community work, staff development, agency-wide projects; serve on agency committees/teams, attend meetings and perform other miscellaneous duties as requested.
  • Perform other duties as assigned.

  • Professional approach to all functions and relationships with all levels of staff, patients, other agencies and the public.
  • Abides by all agency and/or professional code of ethics.
  • Must have strong organizational skills & attention to detail; good oral & written communication skills and demonstrate strong skills in diagnostic and clinical assessment and interventions.
  • Must be able to handle crisis situations in a professional and timely manner.
  • Must be able to work independently as well as part of a team.

Requirements

  • Bachelor’s Degree in Social Work or a related field required. 
  • LSW, LISW, PC, or PCC licensure preferred.
  • Must be a health officer of health officer eligible within 60 days of employment.
  • Must not have a criminal record that would exclude you from working in a behavioral healthcare environment or with law enforcement
  • Must be eligible to drive company vehicles and have a valid driver’s license and be eligible for coverage as defined by the agency commercial insurance carrier.
