Zenex Partners is Hiring a RN Case Manager Near San Pedro, CA

Job Title: Case Manager

Job Location: San Pedro, CA

Shift Timing: Day 5x8-Hour (08:00 - 16:30)

Duration: 13 Weeks

Job Type: Onsite

  • Assessment: Conduct comprehensive assessments of patients to determine their healthcare needs, including medical, psychological, and social factors.
  • Care Planning: Develop individualized care plans that address the unique needs of each patient. These plans consider the patient's medical condition, treatment options, and available resources.
  • Coordination: Collaborate with healthcare professionals, including physicians, specialists, therapists, and social workers, to coordinate and facilitate patient care.
  • Patient Advocacy: Advocate for the best interests of the patient, ensuring they receive quality care and access to necessary services.
  • Communication: Maintain clear and open communication with patients and their families, keeping them informed about their care plans, treatment options, and progress.
  • Resource Management: Identify and utilize available healthcare resources efficiently, including insurance benefits, community services, and support programs.
  • Documentation: Keep detailed and accurate records of patient assessments, care plans, and interactions to ensure compliance with healthcare regulations and standards.
  • Discharge Planning: Plan for patients' safe and timely discharge from the healthcare facility, ensuring they have the necessary support and resources for a smooth transition.
  • Utilization Review: Monitor the appropriateness and efficiency of medical services and treatments to minimize unnecessary costs and promote quality care.

Job Types: Full-time, Contract

Pay: $61.78 - $65.00 per hour

Expected hours: 40 per week
