We’re looking for an experienced RN for part-time Per Diem PRN assignments doing admission assessments, and recertifications in Sussex County.

Ideally you will have at least one years of recent experience as a home health RN.

Responsibilities

• Performing in-home Nursing Assessments
• Conducting Client Re-Certifications every 60 days
• Conducting Caregiver Supervisory visits every 60 days
• Manage the client needs and expectations with regards to the services we provide
• Liaising with Case Manager relating to clients as needed
• Providing Plans of Care to Caregivers

Qualifications

• Must be a registered nurse (RN) licensed in the state and have worked as an RN for at least two years
• A minimum of one year of home health care or equivalent experience is required
• Tech Savy: Essential
• Microsoft Office 365, Outlook, Excel, Word.
