This position is advertised from the minimum to the mid-point hourly rate. Salary will be determined based on education and experience at the time offer.
NATURE OF WORK
(Illustrative Only) Assists with identifying, recruiting and training high-risk individuals to attend health care classes; maintains class records, rosters, registration and follow-up forms; teaches culturally appropriate, effective health promotion and disease prevention classes and workshops for target population; maintain a current list of community resources for patients and their families to maintain and improve their health status; participate in community health fairs and events, networks with community organizations, schools, and outside agencies to provide information on available public health resources in the community; develops, prepares and conducts community outreach and recruitment to enroll individuals in health department programs; acts as advocate and makes appropriate health /community referrals; attend regular staff meetings, trainings, and other meetings as requested; conduct complete, accurate, and timely data entry about activities and outreach; maintains HIPP compliance at all times; regular and reliable attendance is required; performs other duties as assigned.
Hourly rate for this position has been revised effective 3/4/24.
