Youth Shelters Family Services is Hiring a Rapid Rehousing Program Case Manager Near Rio Rancho, NM

Join our team as a dedicated Case Manager, guiding homeless youth and young families through the journey to stable housing and self-sufficiency.

We are currently seeking a dedicated and compassionate individual to join our team as a Case Manager for our Rapid Rehousing Program located in Rio Rancho, NM.

Job Responsibilities:

  • Conduct in-depth assessments to identify and address housing barriers faced by homeless youth and young families, tailoring interventions to meet their unique needs.
  • Serve as a liaison between property managers and program participants, ensuring smooth communication and resolving issues, while fostering positive relationships.
  • Develop personalized housing and financial management plans with clients, aligning with their goals and eligibility criteria, and provide ongoing support and planning.
  • Conduct proactive home visits to monitor progress and offer assistance, mobilizing resources to support participants' community reintegration.
  • Assist participants in creating individualized goal and action plans for permanent housing and self-sufficiency, while maintaining a referral network for ongoing support.
  • Assess life skills needs and provide tailored training, such as communication, problem-solving, and household management.
  • Coordinate with educational and vocational programs for participant access, and organize workshops on relevant topics like financial literacy and job readiness.
  • Collaborate with partners to develop recreational and social activities for community engagement and peer support.
  • Provide crisis intervention and support for participants facing housing instability, mental health, or substance abuse issues.
  • Advocate for participants' needs and rights within relevant systems and organizations.

Qualifications:

  • Experience providing life skills training or vocational guidance to vulnerable populations, especially homeless youth.
  • Familiarity with trauma-informed care and motivational interviewing, with a focus on evidence-based practices.
  • Ability to communicate effectively with diverse individuals and groups.
  • Experience in program development, curriculum design, or workshop facilitation is a plus.
  • Knowledge of community resources and social service systems related to housing, education, employment, healthcare, and mental health.
  • Strong interpersonal skills, including empathy, active listening, conflict resolution, and adaptability to changing priorities.
  • Ability to work independently and collaboratively within a team environment.

What are the minimum education requirements?

  • Bachelor Degree in Human Services preferred
  • OR a two years degree in Human related services and at least one year related experience.
  • OR a high school diploma or GED and at least two years related experience.

What are the scheduled shift you are looking to fill?

  • Monday through Friday - 8AM - 5PM
