ROLE AND RESPONSIBILITIES - Lead Case Manager for the Long-Term Foster Care Program

  • Ensure interagency services are being provided to residents in accordance with the ISP and GOP and are documented and disseminated to applicable parties in a timely and professional manner.
  • Coordination of all services offered to each resident.
  • Provision of case management services as required by interdepartmental standard 22 VAC 40-151-700 for four UIM cases.
  • Lead LTFC Case Manager provides one-on-one supervision to assigned Case Managers and reviews their cases to ensure appropriate language, timely completion of reports and tasks, and to provide ongoing training as needed.
  • Participation in the development, facilitate the meetings, writing the reports and monitor all service plans, including the production of ISP’s and Case Reviews based on standard protocol input from the Office of Refugee Resettlement.
  • Participation in Evaluation Team decisions.
  • Ensure planning process is oriented toward life skills, educational enrichment, preparation for age out (if applicable), safe family reunification, preparation for return to the country of origin, transfer to a higher level of care or less restrictive setting, or transition to an alternative living arrangement once legal immigration status has been obtained.
  • Consistently document the provision of services in each Unaccompanied Child case file.
  • Maintain direct contact with each assigned child in care with at least one scheduled meeting per week to discuss potential reunification options, life skills, educational support, workforce workshops, and/or legal updates.
  • Coordinate with entities in the field to ensure the ISP/GOP (Goals and Objectives Plan) is successfully executed for each child in care.
  • Maintain constant communication with the child’s family and/or sponsor as well as the legal service provider.
  • Maintain complete and updated electronic case files for the child through FAMCARE and UC Portal.
  • Work with Quality Assurance to ensure child’s files are complete and include detailed case notes, intake, assessment, legal, education, medical, and mental health information.
  • Consult regularly with residential staff, clinicians, and public school staff to ensure child is receiving optimal services and to review and discuss those areas needing attention.
  • Attend conferences, meetings, and/or court hearings as needed.
  • Maintain child records and ensure compliance with Interdepartmental Regulations and generally accepted documentation principles.
  • Ensure timely documentation of all required reports
  • Keep track of incoming LTFC referrals, distinguishing between internal and external referrals while also sending out documents to all parties for review.
  • Ensure timely notification of appropriate personnel, parents, and agencies regarding incident reports, medical/dental emergencies, and other Child Level Events (CLE), as required by the ORR and/or licensing entity.
  • Serve as primary point of contact for public school representatives, attorneys, and ORR representatives as needed.
  • Advocate for the child and keep staff focused on child’s individual progress and goals while updating/referencing the GOP.
  • Attend required meetings/staffing and participate in formal and informal professional training.
  • Pursue professional development opportunities.
  • Report to Director of UIM Clinical Services for job duties, quarterly/annual reports, to assist with other tasks, and for supervision.
  • Attendance at all training mandated by YFT and respective governing entity to include but not limited to maintaining current CPR/First Aid certification, Handle with Care certification, and annual TB test/screening.
  • All duties and responsibilities will support meeting the structured program of care outlined in DBHDS and DSS state standards (DBHDS: 12VAC 35-46-800; DSS: 22 VAC 40-151-720).
  • Other duties as assigned by the CEO, Vice President of Programs, Associate Vice President of Programs, Director of UIM Case Management Reunification Services

REQUIRED TRAININGS: ALL TRAINING REQUIRED BY YFT, GOVERNMENT AGENCIES AND LICENSING AND PLACING AGENCIES

  • Master’s Degree in social work, psychology, or counseling or;
  • A baccalaureate degree in social work or psychology with documented field work experience and supervision by the program director or other staff employed by the provider with the same qualifications as required by 22 VAC 40-151-290 D or;
  • A baccalaureate degree and three years of professional experience working with children.
  • Current and maintained CPR, First Aid, and Handle With Care certification.
  • Current and maintained annual TB tests/screenings.
  • Knowledge of Standards for Interdepartmental Regulation of Children’s Residential Facilities and program services.
  • Excellent organizational skills.
  • Ability to communicate well with agencies, parents, and youth.
  • Fluent in English and Spanish
  • Writing proficiency in English and Spanish

CORE COMPETENCIES

  • Knowledge of DSS and ORR policies and regulations.
  • Ability to provide individual supervision.
  • Ability to help facilitate weekly federal staffing in absence of Lead Therapist Clinical Services
  • Knowledge of required weekly, monthly, and quarterly reports
  • Knowledge of all YFT policies and procedures in order to help staff in complying with these
  • Experience with successful completion of reunification process per ORR guidelines.
