Your Health Organization is Hiring a Health Coach Near Myrtle, SC

We are seeking a motivated and compassionate Health Coach's to join our proactive healthcare practice. The primary responsibility of the Health Coach will be to support chronically ill patients, many of whom are homebound, in improving their overall health and well-being. The Health Coach will collaborate closely with the patient's care team, including physicians, nurse practitioners, physician assistants, pharmacists, social workers, registered nurses, behavioral specialists, physical and occupational therapists, dieticians, and other specialists, to develop personalized care plans and provide coaching on health options and interventions.

This is a salary-based position. Schedule: 8am-5pm (Monday-Friday). Must live in the Grandstrand area.

Responsibilities:

  • Collaborate with the patient's care team to develop individualized care plans that address the patient's unique needs, goals, and challenges.
  • Provide coaching and guidance to patients on lifestyle modifications, including diet, exercise, and medication adherence, to improve overall health outcomes.
  • Conduct regular assessments of patients' health status, using a combination of remote patient monitoring, telemedicine, and in-person visits, to evaluate progress and adjust care plans as necessary.
  • Educate patients on disease management strategies, self-care techniques, and preventive measures to promote a higher quality of life.
  • Facilitate communication and coordination among the patient's care team members to ensure a comprehensive and integrated approach to healthcare delivery.
  • Assist patients in navigating the healthcare system, including scheduling appointments, obtaining necessary referrals, and accessing appropriate resources and support services.
  • Monitor patients' compliance with prescribed medications, treatment protocols, and follow-up appointments, providing reminders and reinforcement as needed.
  • Document patient interactions, interventions, and outcomes accurately and thoroughly in electronic medical records.
  • Stay updated on the latest research, guidelines, and best practices in chronic disease management and health coaching to enhance knowledge and skills.
  • Maintain confidentiality and adhere to all applicable privacy and ethical standards in handling patient information.
  • Positions will be a combination of work from home, office and visits in patients homes, assisted livings and rehab centers.

Qualifications:

  • Bachelor's degree in a related field (e.g., health sciences, nursing, psychology) or equivalent experience. A relevant certification in health coaching is preferred.
  • Proven experience in coaching or counseling individuals with chronic health conditions, preferably in a healthcare setting.
  • Solid understanding of chronic diseases, their impact on patients' lives, and evidence-based interventions for managing them.
  • Excellent interpersonal skills and the ability to establish rapport and trust with patients of diverse backgrounds and age groups.
  • Strong communication skills, both verbal and written, with the ability to explain complex medical information in a clear and concise manner.
  • Proficiency in using technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records.
  • Ability to work effectively as part of a multidisciplinary team and collaborate with healthcare professionals from various specialties.
  • Empathetic and patient-centered approach to care, with a genuine desire to help individuals improve their health and well-being.
  • Flexibility and adaptability to accommodate patients' varying needs and to work in a dynamic and evolving healthcare environment.
  • Knowledge of local healthcare resources, community services, and support networks is a plus.
