Yonkers Center for Rehabilitation and Healing is now hiringan experienced MDS Care Plan Coordinator who is committed to providing the best life and care for our residents. Under the: direction of the Director of Nursing Services, the MDS Care Plan Coordinator is responsible for implementing the federally mandated assessment tool to identify needs / concerns of the resident and to coordinate a plan of rare to assist the resident toward optimal wellness based on the highest practicable level of functioning.
We are located at 115 S Broadway, Yonkers, NY 10701.
