Child First is an evidence-based, early childhood intervention that helps families build strong, nurturing relationships that protect and heal young children from the impact of trauma and chronic stress. Child First uses a two-pronged approach in the family's home: (1) Care coordination that provides wrap-around services and supports for the whole family and (2) Child-Parent Psychotherapy (CPP), an attachment-based, trauma-informed, dyadic intervention. The Child First National Program Office (NPO) oversees and supports the local affiliate agencies, such as Bridgeport Hospital, that are authorized to implement the Child First model.
Summary of Position: The Care Coordinator partners with the Clinician to provide services to families who are referred to Child First. The Care Coordinator plays a role in stabilizing the family system to enhance the clinical intervention provided by the Clinician. The Care Coordinator takes the lead in connecting the child and family with desired, community-based services and supports while maintaining Child First's reflective, relationship-based stance. The Care Coordinator will execute the key responsibilities of training, service delivery, data entry and chart documentation, and adherence to the Child First culture, as outlined below.

EDUCATION
Bachelor's degree in social work, psychology, human services, or related field.
EXPERIENCE
Knowledge of and experience with community-based services and supports in service area. Proficiency with computers/tablets and experience with Word and Excel and web-based applications including electronic client records. Recommended but not required: Experience working with culturally diverse children and families, preferably with children under the age of six. Knowledge of early childhood development, parent education, and individual, family, and community-level risk factors (e.g., poverty, homelessness, maternal depression, domestic violence, substance abuse, and teen parenthood). Experience working in home and community-based settings.
SPECIAL SKILLS
Ability to speak a second language highly valued.
PHYSICAL DEMAND
Willingness to work flexible, non-traditional hours when necessary to meet the needs of families. Available to work a minimum of one evening a week to accommodate a home visit starting no earlier than 5:30 pm. Valid driver's license and use of reliable personal vehicle with appropriate insurance for home visits. Ability to conduct home visits including, but not limited to, walking, climbing stairs, standing, sitting (in chairs and on floors), bending, and carrying no more than 25lbs.
Additional Information* This is a grant funded position.
