Yakima Neighborhood Health Services is Hiring a Homeless Respite Case Manager Near Yakima, WA

Our Ideal Candidate: We are looking for a candidate that has a passion to work with individuals who are homeless or in transitional housing The technical qualifications are:

  • Associates degree in behavioral or health sciences or related field and two years closely related work experience.
  • Experience as a medical assistant or certificate of CMA or CNA completion, or willingness to learn. Qualifying work experience may be substituted year for year for education.
  • Knowledge of effective case management practices; knowledge of community resources and experience in effective social service delivery. Knowledge of addiction and mental health pathologies.
  • Proficiency in computer operating systems. 
  • Valid Washington State Driver's License and insurance required.
  • Good attention to detail; good written and verbal communication skills.
  • Bilingual Spanish/English desirable.
  • Candidate has not been sanctioned or excluded from participation in federal or state healthcare programs by a federal or state law enforcement, regulatory, or licensing agency.

Day to Day:

  • Prepare patients for intake to medical respite program. 
  • Collect patient histories, administer vital signs, review client participation agreements and perform other intake preparation as described on respite intake procedures.
  • Conduct exit survey information when patients leave respite program.
  • Prepare to receive patients to medical respite, including arranging for meals, coordinating with YNHS housekeeping, nursing, behavioral health, etc.
  • Coordinate and arrange patient services to primary and specialty care as directed by patient care plan and requested by medical staff.
  •  Document information accurately using appropriate forms and charting format in health care information record.
  • Document information in Homeless Information Management System (HMIS).
  • Assist with patient follow‑up activities, including referrals, recall and other patient needs.
  • Assist with supply management and appropriate stocking of medical respite units.
  • Maintain proper sanitation standards, according to YNHS procedure, both with cleaning and sterilization procedures of patient facilities and instruments.
  • Work with housing case managers to transition patients from respite program to stable housing or alternative when stable housing is not possible.
  • Provide case management support, including identifying needs to address other Social Determinants of Health.
  • Assess need to determine client limitations and strengths.
  • Work with agencies / organizations to maximize clients’ abilities to stabilize housing and improve self-sufficiency.
  • Develop service plan with clients to set goals (including self- management goals) and action steps toward self-sufficiency.
  • Maintaining confidentiality of client interactions and medical records.
  • Participate in staff in-service meetings.
  • Participate in quality assurance activities and peer review as requested.
  • Maintain organized, clean, efficient and confidential work area.
  • Perform related work and other tasks as requested.
