Wound 360 is seeking a highly skilled and compassionate Wound Care QA/Case Manager to join our healthcare team. The ideal candidate will have extensive experience in home health/hospice care management, wound care management, excellent communication skills, and a passion for providing high-quality patient care. As a Wound Coordinator, you will be responsible for coordinating and overseeing the treatment and care of patients with complex or chronic wounds, ensuring optimal outcomes and patient satisfaction.

We have immediate openings for passionate MA/LVN/RN providers, highly motivated who are willing to help patients within Rancho Cucamonga.

The successful candidate will be part of the team, responsible for conducting initial assessments and ongoing evaluations of patient wounds, create tailored care plans and completing progress notes in a timely and efficient manner using a proprietary EMR, working from Monday to Friday.

**Key Responsibilities:**

  • Conduct comprehensive assessments of patients' wounds, including thorough evaluations of wound characteristics, underlying causes, and contributing factors.
  • Develop individualized treatment plans based on assessment findings, evidence-based practices, and patient preferences, in collaboration with interdisciplinary healthcare team members.
  • Coordinate and oversee the implementation of treatment plans, including wound care procedures, dressings, therapies, and interventions, ensuring adherence to best practices and standards of care.
  • Monitor patients' progress closely, evaluating wound healing, assessing for complications, and adjusting treatment plans as necessary to optimize outcomes.
  • Provide patient education on wound care techniques, prevention strategies, self-management skills, and the importance of adherence to treatment plans.
  • Collaborate with physicians, nurses, therapists, specialists, and other healthcare professionals to ensure coordinated and comprehensive care for patients.
  • Advocate for patients' needs, ensuring access to necessary resources, services, and support systems to facilitate healing and improve quality of life.
  • Maintain accurate and up-to-date documentation of patients' medical records, treatment plans, progress notes, and communication with healthcare team members.
  • Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to enhance the delivery of wound care services and promote continuous learning and professional development.
  • Stay informed about current trends, research findings, and best practices in wound care management, and integrate new knowledge into clinical practice.

**Qualifications:**

  • MA/LVN/RN with a valid license to practice in the state of California
  • Certification in wound care management (e.g., Wound, Ostomy, and Continence Nursing Certification) preferred, but not required
  • 1-5 years of clinical experience in wound care management, with demonstrated expertise in assessing and treating complex or chronic wounds.
  • Familiarity of wound care principles, techniques, products, and evidence-based practices.
  • Strong assessment, critical thinking, and problem-solving skills.
  • Excellent communication and patient education skills.
  • Ability to work independently and collaboratively within a multidisciplinary team environment.
  • Proficiency in electronic health record (EHR) documentation and computerized wound care management systems.

**Other Skills & Expectations**

  • Active and unrestricted Driver's License, with reliable means of transportation.
  • Familiarity of wound care principles, techniques, products, and evidence-based practices.
  • Strong experience with Home Health/Hospice Care Management
  • Strong Organizational skill
  • Good supply and inventory management
  • Excellent assessment, critical thinking, problem-solving, and decision-making skills.
  • Effective communication and interpersonal skills, with the ability to collaborate effectively with interdisciplinary team members, patients, and families.

Job Type: Full-time

Pay: $28.00 - $30.00 per hour

Expected hours: 40 per week
