The ideal candidate will have extensive experience in home health/hospice care management, wound care management, excellent communication skills, and a passion for providing high-quality patient care. As a Wound Coordinator, you will be responsible for coordinating and overseeing the treatment and care of patients with complex or chronic wounds, ensuring optimal outcomes and patient satisfaction.

We have immediate openings for passionate MA/LVN/RN providers, highly motivated who are willing to help patients within San Bernardino.
The successful candidate will be part of the team, responsible for conducting initial assessments and ongoing evaluations of patient wounds, create tailored care plans and completing progress notes in a timely and efficient manner using a proprietary EMR, working from Monday to Friday.
