Wound360 is Hiring a LVN Case Manager / Quality Assurance Near Ontario, CA

Wound Masters Medical Group, Inc. is a group of dedicated surgeons, physicians, PAs and NPs who are especially trained to provide mobile wound care and gastric feeding tube replacements at the patient's residence.

We are seeking a qualified and experienced Mobile Wound Care Case Manager to join our dynamic healthcare team. As a Wound Care QA / Case Manager, you will be responsible for coordinating comprehensive wound care services to patients in various settings, including homes, long-term care facilities, and other community-based locations. The role involves coordinating, evaluating and communicating wound care interventions to ensure optimal patient outcomes.

Responsibilities:

1. Patient Assessment:

  • Conduct thorough assessments of patients medical records regarding and everything that relates to wounds, considering the individual's medical history, current health status, and environmental factors.
  • Gather and document important notes that will aid with the provider’s wound assessments and provision of treatment.

2. Care Planning:

  • Coordinate personalized wound care plans based on the assessment findings, medical history, and patient goals.
  • Collaborate with physicians, nurses, and other healthcare professionals to establish comprehensive care plans.

3. Wound Care Interventions:

  • Provide direct wound care support as needed, including wound dressings, and other advanced wound care techniques.
  • Educate patients, caregivers, and other healthcare staff on proper wound care techniques and preventive measures.
  • Travel to patients' homes, long-term care facilities, and community settings to provide front and back office support and assist the provider.
  • Ensure all equipment and supplies necessary for wound care are available and in working order during mobile visits.

4. Coordination of Care:

  • Collaborate with other healthcare providers, including primary care physicians, specialists, and home healthcare agencies, to ensure coordinated and effective care.
  • Communicate regularly with patients and their families to provide updates, answer questions, and address concerns.

5. Quality Assurance Oversight:

  • Develop and implement quality assurance protocols and guidelines specific to mobile wound care services.
  • Regularly review and update quality assurance policies to align with industry standards and best practices.
  • Conduct internal audits to ensure compliance with regulatory requirements and adherence to established protocols.

6. Documentation and Record Keeping:

  • Monitor and evaluate documentation practices to ensure accuracy, completeness, and timeliness of patient records.
  • Collaborate with healthcare providers to improve documentation processes and address any deficiencies.
  • Maintain a comprehensive record of quality assurance activities, audits, and outcomes.

7. Education and Training:

  • Conduct educational sessions for patients, healthcare professionals, caregivers, and patients on wound care best practices, prevention, and management.

8. Quality Improvement:

  • Participate in quality improvement initiatives to enhance the delivery of wound care services.
  • Stay informed about the latest advancements in wound care and incorporate evidence-based practices into daily routines.

Requirements:

  • Active California Licensed Vocational Nurse (LVN) License
  • Certification in Wound Care (WOCN, CWOCN, or equivalent) preferred.
  • Minimum of 1 year of clinical experience with a focus on wound care.
  • Minimum of 5 years healthcare experience with computer know-how and access to Electronic Medical Records
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Valid driver's license and reliable transportation.

Job Type: Full-time

Pay: $28.00 - $30.00 per hour

Expected hours: 40 per week
