Woundmasters is seeking a highly skilled and compassionate Wound Care Coordinator to join our healthcare team. The ideal candidate will have extensive experience in home health/hospice care management, wound care management, excellent communication skills, and a passion for providing high-quality patient care. As a Wound Coordinator, you will be responsible for coordinating and overseeing the treatment and care of patients with complex or chronic wounds, ensuring optimal outcomes and patient satisfaction.

We have immediate openings for passionate MA/LVN/RN providers, highly motivated who are willing to help patients within Covina.
The successful candidate will be part of the team, responsible for conducting initial assessments and ongoing evaluations of patient wounds, create tailored care plans and completing progress notes in a timely and efficient manner using a proprietary EMR, working from Monday to Friday.

**Qualifications:**
- MA/LVN/RN with a valid license to practice in the state of California
-Certification in wound care management (e.g., Wound, Ostomy, and Continence Nursing Certification) preferred, but not required
-1-5 years of clinical experience in wound care management, with demonstrated expertise in assessing and treating complex or chronic wounds.
-Familiarity of wound care principles, techniques, products, and evidence-based practices.
- Strong assessment, critical thinking, and problem-solving skills.
- Excellent communication and patient education skills.
- Ability to work independently and collaboratively within a multidisciplinary team environment.
- Proficiency in electronic health record (EHR) documentation and computerized wound care management systems.
