Required:
* Valid Oregon Driver's License, insurable driving record; personal automobile insurance
* Maintain CPR/First Aid certification
* Meet the requirements to become MHACBO QMHA Certified - certification must be maintain for the length of employment (Must meet ONE of the following to qualify - Bachelor's degree in a behavior science field OR a combination of at least 3 years relevant work, education, training, or experience)
* Communicate effectively
* Understand mental health assessment, treatment & service terminology & apply each of these concepts
* Implement skills development strategies & identify, implement & coordinate the services & supports identified in an ISSP

Preferred:
* Previous experience working with adults diagnosed with mental illness or any disability group

Summary:
Responsible for assisting in the care & treatment of residents (residential, respite & hold), including life skills training & limited psycho-social interventions. Performs a wide variety of paraprofessional duties to assist in delivering services to the mentally ill adults. Facilitates resident cooking, cleaning & general maintenance duties. May be assigned in certain settings to administer medications & perform basic nursing duties under the supervision of the Registered Nurse (Division 47 of the Nurse Practice Act.)

Duties Include:
* Read & understand each resident's individual service plan
* May be assigned to assume a temporary lead role
* Participate with the health care team in selecting appropriate candidates for residence
* Provides discharge planning for residential & respite clients
* Completes admission & /or discharge process & paperwork for residential, respite & hold clients
* Participate in Residential Service Planning & acts as member of the interdisciplinary treatment team
* Provide therapeutic interventions with residents as prescribed by the Residential Service Plan
* Observe & monitor residents & document accordingly in the resident's record as prescribed by the Residential Service Plan
* Interacts with other members of the health care team
* Directs & assists residents & actively participates in facility meal preparation, cleaning, laundry & general maintenance duties
* Plan, coordinate & supervise recreation activities & outings for residents
* Transport, accompany & actively supervision residents as necessary or assigned
* Monitor safety & security of the environment
* Audit resident charts, documents services & activities in each resident's electronic health record
* Answer telephones
* Inform Supervisor or Program Administrator of resident & facility needs & concerns
* Maintain positive work environment & working relationships with peers, medical & rehabilitation staff
* Attend & actively participate in general staff meetings
* Maintain timely, complete written records of resident contacts, attendance & other documentation required by state & county regulations for residential care

Pay: $16.59 - $24.89 per hour
