Job Description: As an LPN (Licensed Practical Nurse) in care coordination setting or as an intake person, you'll be responsible for assessing patients' needs, developing care plans, and coordinating healthcare services to ensure continuity of care. This role involves collaborating with healthcare professionals, patients, and their families to optimize patient outcomes and promote wellness. Effective communication, critical thinking, and the ability to prioritize tasks are crucial skills for success in LPN care coordination.

• Healthcare delivery experience.
• Willing to consider and train recent grads.
• Strong organizational skills and ability to think clearly and to work in a busy clinic.
• Strong skills and proficiency in OncoEMR.
• High School Diploma and graduation from an approved School of Practical Nursing.
• Valid LPN License in New York State, Infection Control Certificate, & Child Abuse Certificate

Pay Rates: *Private Benefits

• Range of $62,400-$83,200 annual salary possibilities or $30.00-$40.00 hourly rate based on experience with Full Time Benefits.

Shifts:

• Monday-Friday / 8.5 Hours work day during Hours of Operation = 8am - 6pm.

Requirements (MUST HAVE & Valid/Up to date/Current):

• NYS LPN Licensing / Certification VALID
• BLS/CPR
• Bilingual Spanish a PLUS / Español bilingüe preferido O Español bilingüe preferida!
• Background screening [at least Passed a previous screening]
