Job DescriptionSupported Living Case ManagerFull Time (40 hrs.) at $23.00phr – Quincy AreaWORK Inc., one of New England’s most progressive providers of services to adults with disabilities is seeking a Supported Living Case Managers who will support individuals served in our Supported Living Program by promoting independence while empowering and enhancing their quality of life.
Responsibilities will include assisting the client with transportation, attending medical appointments, and daily living activities such as grocery shopping and participating in community activities. Case managers maintain a small caseload of clients allowing them to build and foster a person-centered relationship with each person they support.
Preference will be given to candidates that have experience working with individuals with developmental disabilities and/or mental health and are familiar with DDS regulations and ISP process. The ideal candidate for this position is a self-motivated, energetic, team-player who enjoys working with and collaborating with various professionals, diverse individuals served, and family members.
Hours:Full Time position is 40hrs per week, Monday to Friday, 9am- 5pm (some flexibility available).
This position includes frequently visiting individuals’ homes throughout our service area.
Benefits: This is a $23.00/hour position.
Holidays off and yearly vacation accrual.

Minimum requirements:
  • Possession of a valid driver's license and acceptable driving record. Must have own transportation and must be willing to transport individuals in own vehicle.
  • College degree in Psychology, Sociology, Rehabilitation Counseling, Special Education, Social Work or related field and/or a minimum of one plus years’ experience or High School Diploma with 2 years of experience in a related field.
