A Community Resource Specialist (CRS) at Wood River Health is responsible for helping patients and their families navigate and access community services, resources, and support. The CRS works closely with providers and the Director of Care Coordination through an integrated approach to care management and community outreach. As a priority, activity will promote, maintain, and improve the health of patients and families as well as provide social support and advocacy for individuals and community health needs. Community outreach such as home/community visits will be required in accordance with the policies, procedures and philosophies of Wood River Health. Essential Duties include:
