A Community Resource Specialist (CRS) at Wood River Health is responsible for helping patients and their families navigate and access community services, resources, and support. The CRS works closely with providers and the Director of Care Coordination through an integrated approach to care management and community outreach. As a priority, activity will promote, maintain, and improve the health of patients and families as well as provide social support and advocacy for individuals and community health needs. Community outreach such as home/community visits will be required in accordance with the policies, procedures and philosophies of Wood River Health. Essential Duties include:

  • Research resources and programs available to patients and their caregivers but not limited to:
    • Conduct outreach in the community that promotes, maintains and improves individual and community health
    • Provide information on available resources, provide social support and advocate for individual and community health needs
    • Establish and maintain relationships with community agencies and organizations to meet community needs and to ensure services are not duplicated
    • Help patients connect with resources and give appointment reminders in special circumstances
    • Provide referrals for services to community agencies as appropriate
    • Identify patient’s needs indicated through Social Determinants of Health screenings and appropriately refer patients to various resources, tracking the progress of referrals in accordance with Wood River Health policies
  • Work collaboratively to support patients. This may include but is not limited to:
    • Work closely with care teams (medical, BH, Dental) to help ensure that patients have comprehensive and coordinated care. Follow-up with patients should be continuous from initial identification through closure
  • Patient Education and Advocacy
    • Link patients to appropriate community resources including but not limited to information/referral sources for financial assistance, transportation, support groups and other community services
    • Follow-up with patients via phone calls, and conduct site, home or community visits
    • Conduct intake interviews with patients, including enrolling and/or referring patients to programs
    • Program enrollment assistance (including by not limited to completing coverage applications, gathering required documentation and troubleshooting the enrollment process) for uninsured children and adults to access subsidized, low-cost or free health insurance program through the health insurance marketplace (CT & RI), Medicaid and Rhode Island RIteCare
    • Provide sliding fee assistance when applicable
    • The CRS will be responsible for tracking of the following, but not limited to: Enrollments, Sliding Scale, referrals and PCMH
  • Other Responsibilities:
    • Exhibit excellent working relations with patients, visitors and staff
    • Responsible for providing consistent communication to the Manager of Care Coordination to evaluate patient/family status, ensuring that reports and information provided clearly describe progress
    • Collect data to identify community health needs
    • Maintain data for required reports
    • Record and maintain patient care management information in the EHR, no later than 24 hours after patient contact
    • Attend case review meetings and pre-visit planning, as needed
    • Manage assigned caseload of patients
    • Maintain strict confidentiality in all matters
    • Operate within the scope of the Health Information Portability and Accountability Act to safeguard the privacy of protected patient health information
    • Enliven and support the mission, vision, and values of WRH
    • Adhere to organizational policies and procedures
    • Adhere to the WRH Compliance Program Standards
    • Perform other duties as assigned to meet business needs

Qualifications:

  • Bachelor’s degree in Social Work, Human Services or related field preferred, or 3 years of equivalent experience.
  • Must obtain Health Source RI certification at first available training dependent on RIHCA training schedule and must maintain annual re-certification.
  • Previous experience with Community-Based Services, preferred.
  • Previous experience working in a Community Health Center or healthcare, preferred.
  • Experience with Electronic Health Records, preferred.
  • Proficient computer skills
  • Cultural sensitivity necessary to work with a diverse patient and staff population.
