The Community Care Coordinator performs administrative and customer service functions associated with the Community Resources department. Activities include non-clinical administrative support to the care management staff; provides assistance with utilization management (Emergency room/Urgent care, and hospitalizations), and processes to maintain compliance with federal, state and managed care guidelines. This position facilitates the scheduling of patients for available and appropriate transportation services. This position will provide assistance with the maintenance of our data integrity. Essential Duties include:

  • Retrieve data daily including but not limited to: referrals, labs, tests, and hospitalizations/ER visits, from appropriate sources including but not limited to; Current Care Viewer, Direct Messaging, Rhode Island Quality Institute Dashboard and Hospital Provider Portals. Adding relevant data into the EHR.
  • Facilitate follow up care for patients who recently visited the Emergency Department in compliance with the health center’s accountable entity.
  • Assess patients for any gaps in care and schedule appointments as appropriate
  • Assess patients for transportation needs, if patients need transportation schedule appropriately to transportation services
  • Collaborates closely with the care team to coordinate visits, daily workflow, and visit planning preparation.
  • Conduct over the phone assessments of patients for social needs (SDoH) and income data, educates members on after-hours, same-day, and social services available through Wood River Health
  • Retrieve daily PRAPARE reports and generate appropriate referrals to Outreach team.
  • Performs a variety of specific clerical tasks to support Care Management Department.
  • Documents clinical information, including patient encounters, accurately and completely.
  • Assists with special programs including data collection for process improvement projects.
  • Educate patients on the patient portal and Current Care. Assist with enrollment as needed.
  • Maintains relevant information database and provides monthly data reports to include orderly analysis of data and program assessment for measurement and reporting the impact of the plan of care to the healthcare delivery team and for quality management tracking.
  • Coordinate patient services for special programs as required
  • Enliven and support the mission, vision and values of WRH.
  • Adhere to organizational policies and procedures.
  • Adhere to the WRH Compliance Program Standards.
  • Performing other duties as assigned to meet business needs.

Qualifications:

  • High School Diploma or equivalent.
  • Experience in scheduling patients for services or dispatch experience, required
  • Experience working in a community health center, preferred
  • Knowledge of Patient Centered Medical Home an
  • Microsoft Office (excel, PowerPoint and word required)
  • Certification by the National Association of Medical Assistants Or Community Health Worker Certification, preferred
