The Community Care Coordinator performs administrative and customer service functions associated with the Community Resources department. Activities include non-clinical administrative support to the care management staff; provides assistance with utilization management (Emergency room/Urgent care, and hospitalizations), and processes to maintain compliance with federal, state and managed care guidelines. This position facilitates the scheduling of patients for available and appropriate transportation services. This position will provide assistance with the maintenance of our data integrity.
