What you will do:

As a Nurse Case Manager you will oversee and help manage the delivery of care from pre-admission to post-admission. responsible for interacting/updating HMO’s with weekly progress with therapy and nursing interventions. Also responsible for managing Medicare-recipients on caseload. Must have good clinical skills and assessment ability, and work with I tear-disciplinary team.You will help ensure residents and their families receive appropriate utilization of services and coordination of care.

Must haves:

• Successfully graduated from an approved and certified RN/LVN program
• Current and unencumbered license to practice as an RN/LVN in the state
• Ability to make quick judgements and work independently
• Must be knowledgeable of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care and Geratrics.

Would be great:

• 1 year experience as an RN/LVN
• Case Management 1 year (preferred)
• RN (preferred)

Compensation for roles at Windsor Elmhaven Care Center varies depending on a wide array of factors including but not limited to the location, role, skill set and level of experience.As required by state or local law, Windsor Elmhaven Care Center provides a reasonable pay scale to include the hourly or salary range that Windsor ELmhaven Care Center reasonably expects to pay for roles that may be hired in California, as stated below:

Pay Range: $35 - $45 per hour Depends on the Experience
