Wind River Family & Community Health Care is Hiring a Case Manager Near Riverton, WY

DEPARTMENT: Population Health
REPORTS TO: Case Management Department Head
SALARY: D.O.E.
JOB TYPE: Full-Time/Non-Exempt/Permanent/Some evenings and weekend hours

JOB OVERVIEW:

Develops individualized care plans, monitor outcomes, and re-assess throughout the process. This will include screening for strengths, risks, and needs. Through individualized care plans wrap-around care will be provided to patients. These services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care.


DUTIES AND RESPONSIBILITIES:

Completion of checklists, summary reports, data entry and assistance in the creation of case plans.
Assist in organizing services and support as an integrated system of health care for vulnerable and in need of care patients.
Assist patients in accessing support and services as specified on individual plans of care.
Provide input and recommendations for treatment planning.
Document all contacts and services in narrative form, enter data into the appropriate database, and obtain all required signatures for service delivery.
Establish and maintain quality relationships with agencies, community, and other resources to facilitate appropriate referral recommendations.
Provide ancillary support services to groups and individuals and implementation of evidenced-based and culturally best practice services.
Assist in facilitating education groups.
Other duties as assigned are to maintain and enhance the operation of the department.
Performs other duties as assigned to maintain and enhance the operation of the program.


QUALIFICATIONS:

Bachelor’s in social sciences.
Or previous employment as a Case Manager with 5 or more years of experience in the provision of case management or direct care plan service delivery to patients in the social work/health/juvenile justice field.
Or Registered Nurse with 2 years of case management experience.
Or an associate degree in criminal justice/human services/early childhood education and two years related experience in case management/direct care service delivery.
Communication skills are required to effectively coordinate services between Wind River Family & Community Health Care (WRFCHC) and entities serving the Wind River Reservation.
Must have the ability to compile and write reports from various documents maintained within WRFCHC is essential.
Must have the ability to work in stressful situations and must be highly motivated to ensure the highest possible quality of care is made available to patients referred for services.
Must possess a valid driver license and be eligible for coverage under WRFCHC’s auto insurance policy.
Must be able to pass a criminal background check under the Indian Child Protection Act and Family Act (P.L.101-630).
