**Job Overview**
Willow Health Care is seeking a detail-oriented MDS Coordinator to join our healthcare team. This is a corporate MDS coordinator positon. This position will oversee the MDS assessment for our five skilled nursing facilties. The MDS Coordinator will be responsible for conducting assessments, developing care plans, and ensuring compliance with regulations.

**Duties:**

- Conduct comprehensive assessments of patients' health status

- Develop individualized care plans based on assessment findings

**Experience:**

-RN minimum two years experiance MDS

-RAC certfied preferred

-Working knowledge of PDPM and Missouri Case Mix
