White Sands Treatment Center is a leading provider of drug and alcohol rehabilitative services in Florida. We provide evidence-based treatment in a residential setting for adult men and women.
We are seeking a strong professional to join our team as a Case Manager for our facility in Plant City, FL. The Case Manager is responsible for facilitating admissions, discharges, and to affect a safe and supportive environment for patients.
Responsibilities
Responsible for facilitating the admission process.
Notify nursing of all B2B transfers for nurse-to-nurse report and eyes on
Responsible for coordinating arrival with all departments.
Responsible for collecting all payments and executing Financial Docusigns
Responsible for making “safety calls” for all admissions.
Responsible for entering pending admissions into KIPU and completing Financial Packets as needed
Meet with all patients regarding admission or financial related concerns.
Receive medical records for pending admissions and provide to medical director or admission’s office.
Update Coordination of Benefits for all applicable parties
Facilitate tours for families and potential admissions
Responsible for regular follow up meetings with caseload
Responsible for touring patients and providing program overview
Providing orientation by group schedule, therapist assignment, CM and back up CM to all patients after 24 hours
Responsible for documenting all patient meetings in Electronic Medical Record and tracking sheets
Monitor group attendance and address if needed
Monitor progress/behaviors and redirect if needed
Communicate with treatment team on progress, issues, discharge plans
Facilitate Behavior Contracts as needed
Communicates and deals effectively with patient’s family members and referral sources
Communicates and problem solves via the chain of command.
Responds to incoming telephone inquiries and initiates follow-up communications.
Act as a prime source of information for court contact and the general public.
Responsible for handling patient concerns and complaints in a timely, professional manner
Responsible for de-escalation and crisis intervention as needed
Communicate Crisis/ AMA/ Incidents to Management
Responsible for facilitating discharge of patients to include arranging transportation
DC planning halfway through patient’s stay
Responsible for scheduling aftercare appointments and arranging step-down transfers
Communicate discharge plans/ changes to QA Director and assigned therapist
Responsible for follow up post completion of treatment (alumni)
Attends departmental meeting daily
Employer Related affairs– FMLA, STD, LOA, COBRA
Attends in-service training, when necessary.
Completes other tasks, as assigned
Ensure that documentation is timely, consistent, clear and articulate
Keep clinical records secure at all times and maintain confidentiality pertaining to clients and families
Will accept and utilize supervision
Address problems noted by supervisor
Will communicate with supervisor and keep supervisor informed of significant case developments
Participate in Quality Improvement activities and contribute to the welfare of the organization
Pursue professional development through participation in education and training programs
Complete Incident reports as required.

Demonstrate good strategic and problem-solving skills
Have good work habits: complete tasks in a timely and efficient manner, adhere to time schedules, maintain good communication, demonstrate willingness to accept responsibility and function well as a team member and contribute positively to the morale of staff
Use verbal and non-verbal communication with others, i.e., courtesy, tone of voice, facial expressions, gestures, etc.
Demonstrates excellent organizational skills and originator of new ideas and methods.
Education/Experience:
High School Diploma or GED, bachelor’s degree preferred / 2 years of experience working in the substance abuse or mental health field. Certfied Case Manager preferred
