Job Summary: Identifying Primary Care Physicians for patients, lining up transportation needs, ordering durable medical equipment, sending referrals to post-acute care agencies (nursing homes, home health companies), verifying guardianships and ensuring the needed documents are reflected in the Electronic Medical Record, delivering/explaining the Important Message from Medicare (IMM) to Medicare Patients, delivering/explaining the MOON (OBS brochure) notice to observation patients. As a Case Management Assistant you would work independently, but always would be working under the supervision of an RN or Social Worker Case Manager.

Qualifications: Prefer to hold at least a 2 year college degree and have some experience in healthcare, or be a student of a healthcare related field.
