Wheeler Mission is Hiring an Emergency Services (ESS) Case Manager Near Indianapolis, IN

Position Summary:

This position is responsible for providing a continuum of care for Emergency Shelter guests and program participants through case management, including a thorough assessment and identification of each guest’s resources, talents, skills and interests including a plan to identify and achieve personal goals. The ESS Case Manager is also responsible for completing intakes with new Center for Women and Children (CWC) guests.

Wheeler Mission (WM) is a privately funded 501 (c) 3 non-profit, evangelical Christian ministry whose designated purpose is religious. We are a Christ-centered ministry which is dedicated to sharing the Gospel and helping the homeless and impoverished. Qualifications

Qualifications:

To perform the essential functions of this position successfully, an individual must demonstrate the following competencies:

  • Possess a sincere love of Christ and maintain a Christian walk that is evident of that love. Individual must be willing to sign and adhere to the WM Statement of Faith.
  • Be dependable – consistently at work and on time, report to all scheduled shifts, follow instructions, respond to management direction and solicit feedback to improve personal performance, and complete tasks in a timely and accurate manner.
  • Be flexible and able to work with women experiencing homelessness from various backgrounds and at different skill levels.
  • Be willing to learn and navigate community housing resources and help guests connect to housing.
  • Be comfortable with utilizing a combination of best practices and faith-based concepts to serve guests with excellence.

Essential Requirements:

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Intake: Complete the intake process for incoming CWC guests including all related documentation.
  • Case Work: Provide case management services for a diverse population of women that may include, but is not limited to, assessment of guest’s strengths, resources and needs, crisis intervention, information and referral, all directed toward achieving total stability for each guest. She will meet with assigned guests within 48 hours of their entering the CWC to begin this process.
  • Coordination of Services: She will work collaboratively with other staff members and the guest to help the guest achieve goals. She will collaborate with other agencies to coordinate services designed to build a support network for the guest and serve as an advocate.
  • Planning: Develop a care plan with written goals and methods to help the guest achieve those goals. She will monitor the effectiveness, appropriateness and efficiency of the plan modifying as needed.
  • Record Keeping: Maintain appropriate documentation of guest sessions for identifying information, agency referral and follow-up. She will retain this information on a database that can easily be shared for grant writing and publications when needed.
  • Legal: Take appropriate action according to the established protocol of the organization, maintaining guest confidentiality in all areas following biblical definitions of ethical conduct and behavior.
  • Case Conferences: Participate in multidisciplinary guest conferences that are designed to share information beneficial to the guest.

Work Environment:

The Case Manager must be able to carry out the required job duties in a fast-paced Christ Centered environment that is grace-based, trauma informed, and strengths focused.

Physical Demands:

To perform the essential job functions of the Case Manager position, a candidate must be capable of performing many physical functions including:

1. Walking short distances with the ability to ascend and descend ten flights of stairs

2. Physical dexterity sufficient to type and operate standard office equipment

3. Physical ability to perform normal office functions with the endurance to remain standing for long periods of time

4. Strength to lift office supplies and equipment up to 20 pounds

The Case Manager must be able to:

1. Read, write and interpret written documents

2. Communicate effectively with guests, co-workers, community partners and volunteers

Preferred Education and Experience:

  • BA/BS or the equivalent in education or experience
  • Experience working with women who may have experienced substance abuse, mental health concerns, trauma, domestic violence or homelessness

Preferred Education and Experience:

  • Experience with counseling, mentoring and disciplining women

Background Investigation and Drug Testing:

  • After a “conditional offer of employment” is made, the individual must pass a background investigation which includes a consumer credit investigative report.
  • Wheeler Mission Ministries has a mandatory drug testing policy for all new employees. The successful applicant must submit to and pass a drug and/or alcohol test. This drug and/or alcohol testing may occur any time after the applicant receives a conditional offer of employment. All employees must sign and adhere to WM Drugs and Alcohol policy and remain free from addiction to drugs or alcohol.

Statement of Conflict of Interest/Confidentiality

This position requires both a conflict of interest statement and a confidentiality statement.

Supervisory Responsibilities:

The Case Manager may be responsible for providing task supervision of program assistants, interns, and volunteers.

Expected Hours of Work:

This full-time position is typically staffed throughout the week with some evening and weekend hours required. Schedules are assigned at the discretion of the Director or Program Director and may change at any time with or without advance notice.

Travel:

Local travel to events and vendors will be required.
