We are looking for a dynamic MDS Coordinator, LVN to join our Skilled Nursing Facility or Subacute Care!

RESPONSIBILITIES:

• Complete accurate and timely MDS assessments for all residents
• Collaborate with interdisciplinary team members to develop and revise care plans
• Monitor and ensure the delivery of high-quality resident care
• Coordinate with medical professionals to address resident needs
• Maintain and update resident records and documentation
• Participate in quality improvement initiatives and audits
• Provide education and training to staff regarding MDS guidelines and documentation requirements

REQUIREMENTS:

• Valid and active Licensed Vocational Nurse (LVN) license in the state of California.
• Must have an active BLS through the American Heart Association
• Must have an active CPR Certification
• Bachelor's degree in nursing or a related field
• Is in good standing with all regulatory agencies and licensing boards is required
• Interest in working with the geriatric population
• Strong understanding of MDS guidelines and regulatory requirements
• Excellent attention to detail and organizational skills
• Ability to effectively communicate and collaborate with interdisciplinary team members
• Proficient in electronic health record systems and MDS software
• Knowledge of Medicare and Medicaid reimbursement guidelines is a plus

EXPERIENCE:

• Must have, as a minimum, 1 year experience as LVN in a long-term care facility, or other related health care facility.
• Minimum of 1 year of experience as an MDS Coordinator in a skilled nursing facility
