Atlee Hill Skilled Nursing & Rehabilitation Center is Hiring a Geriatric Nursing Assistant / Sign-on Bonus Starting at $3K Near Westminster, MD

PURPOSE OF POSITION:
The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as may be directed by your supervisors
DELEGATION OF AUTHORITY:
As a Geriatric Nursing Assistant you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.
JOB FUNCTIONS:
Every effort has been made to identify the essential functions of this position. However, it in no way states or implies that these are the only duties you will be required to perform. Duties and Responsibilities
Administrative Functions:
  1. Use the wristband or photo card file to identify residents before administering ADL care, treatments, serving meals, etc., as necessary.
  2. Record all entries on flow sheets in an informative and descriptive manner (i.e. NA Flowsheet, I & O sheet, VS, Intake sheets, etc.).
  3. Use only authorized abbreviations established by this facility when recording information.
  4. Report all changes in the resident's condition, such as change in eating habits, ADL level, level of consciousness, skin breakdown, injury, etc., to the Nurse Supervisor/Charge Nurse as soon as practical.
  5. Report all accidents and incidents you observe on the shift that they occur.
  6. Report any resident or family requests or complaints to the Charge Nurse.
Admission, Transfer, and Discharge Functions:
  1. Ensure that the resident's room is ready for receiving the resident (i.e. bed made, name tags up, admission kit available, etc.).
  2. Greet residents and escort them to their room.
  3. Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate and orient to room.
  4. Make resident comfortable (i.e. put to bed, get water, etc.).
  5. Instruct in use of call light.
  6. Inventory, store, and mark the resident's personal possessions as instructed, completing facility clothing list.
  7. Obtain base line vital signs.
  8. Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged.
  9. Prepare resident for transfer making sure resident is clean, and appropriately dressed.
  10. Transport residents to new rooms or to the receiving area.
  11. 11. Assist with copying of transfer documents if requested by the Charge Nurse.
  12. Assist with loading/unloading residents from vehicles as necessary.
Personal Nursing Care Functions:
  1. Make safety round and note condition of residents on arrival to unit.
  2. Participate in and receive the nursing report from the Charge Nurse on assigned residents.
  3. Follow established policies concerning exposure to blood/body fluids.
  4. Assist residents with daily dental and mouth care (i.e. brushing teeth/dentures, oral hygiene, special mouth care, etc.).
  5. Assist residents with bath functions (i.e. bed bath, tub or shower bath, etc.) as directed and per unit schedule.
  6. Give backrubs as instructed.
  7. Assist residents with dressing/undressing as necessary supporting independent function when possible.
  8. Assist residents with hair care functions (i.e. combing, brushing, shampooing, etc.).
  9. Assist residents with nail care (i.e. clipping, trimming, and cleaning the fingernails). (Note: Does not include diabetic residents). Report to Charge Nurse when toenails need to be cut.
  10. Shave male residents daily.
  11. Keep hair on female residents clean shaven (i.e. facial hair, under arms, on legs, etc.) as instructed.
  12. 12. Keep residents dry (i.e. change gowns, clothing, linen, etc., when it becomes wet or soiled).
  13. Make beds (occupied and unoccupied),using proper body mechanics. Keep linens tight to avoid wrinkles from forming under the resident.
  14. Dispose of soiled personal linens per facility policy.
  15. Position bedfast residents in correct and comfortable position.
  16. Assist resident with bowel and bladder functions (i.e. take to bathroom, offer bedpan/urinal, portable commode, empty urinary collection bags, etc.).
  17. Maintain intake and output records as instructed.
  18. Keep incontinent residents clean and dry.
  19. Maintain bladder and bowel training program per facility policies and document appropriately.
  20. Check and report bowel movements and character of stools as instructed.
  21. Collect specimens as instructed (i.e. urineĀ± sputum, stools, etc.).
  22. Assist residents in preparing for medical tests (i.e. lab work, x-ray, therapy, dental, etc.) as directed.
  23. Assist residents in preparing for activity and social programs (i.e. church services, parties, visitors, etc.).
  24. Assist in transporting residents to/from appointments, activity and social programs, etc., as necessary.
  25. Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., using proper body mechanics.
  26. Check frequently to assure drainage and feeding tubes are free and clear.
  27. Assist residents to walk with or without self-help devices as instructed.
  28. Perform restorative and rehabilitative procedures as instructed.
  29. Provide eye and ear care (i.e. cleaning eyeglasses/hearing aides, etc.) as instructed.
  30. Assist in preparing the resident for a physical examination.
  31. Weigh and measure residents as instructed. Report weight changes immediately.
  32. Measure and record temperatures, pulse, and respirations (TPRs) and blood pressures as instructed. Report any out of normal ranges to Charge Nurse,
  33. Answer resident calls promptly. Knock on door prior to entering. Do not ignore call lights for identified residents not specifically on assignments.
  34. Ensure that residents who are unable to call for help are checked frequently.
  35. 3 Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.
  36. Assist with the care of the dying resident.
  37. Provide post-mortem care as instructed.
Education Requirements:
  • Must have attended and successfully completed a Geriatric Nursing Assistant Program and have proper certification.
  • Must have completed Basic Life Support (CPR and AED) Program, up to date.
